Medical Social Worker

Job Description:
Provides medical social services to patients in their homes in accordance with physician orders and under the direction and supervision of the Director of Clinical Services or another appropriate supervisor.

Job Conditions:

The ability to communicate well, both verbally and in writing is required.

The ability to access patient’s homes which may not be routinely wheelchair accessible is required.

Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and to perform patient care.

Physical activities will include walking, sitting, stooping and standing and minimal to maximum lifting of patients and the turning of patients.

Qualifications:

Master’s Degree from a School of Social Work approved by the Council of Social Work Education

Must have or be in the process of acquiring certification from the Academy of Certified Social Workers or similar certifying organization

Must be licensed in the State of Alaska

Two years of experience preferred, with at least one year of experience in a health care setting (hospital, clinic, rehabilitation center, etc.).

Must have a fingerprint criminal background check

Must have a current CPR certification

Driver License, Car Insurance, TB Skin Test Clearance

Occupational Therapist

Job Description:
Provides skilled occupational therapy service on an intermittent basis to patients in their homes in accordance with physician orders.

Job Conditions:
The ability to drive and the ability to access patient’s homes which may not be routinely wheelchair accessible is required.
Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and to perform patient care.
On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/ or carry up to 30 pounds.

 

Qualifications:

Must be a graduate of an Occupational Therapy school approved by the Council of Medical Education of the American Medical Association in collaboration with the American Occupational Therapy Association.
Preferably should be a member of the American Occupational Therapy Association.
Must be licensed in the State of Alaska.
Must have at least one year of home health or institutional experience.
Must have a fingerprint criminal background check.
Must have a current CPR certification.

Physical Therapist

Job Description:

CAREERS

Registered Nurse

Job Description:

To provide nursing care in accordance with the patient’s plan of care to include comprehensive health and psychosocial evaluation, monitoring of the patient’s condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care.

Job Conditions:

Must have a driver’s license and be willing and able to drive to patient’s residences.

The ability to access patients’ homes which may not be routinely wheelchair accessible is required. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and to perform and demonstrate patient care.

Physical activities will include walking, sitting, stooping and standing and minimal to maximum lifting of patients and the turning of patients.

The ability to communicate both verbally and in writing is required as frequent communication by telephone and in writing is involved.

Qualifications:

Must be a graduate from an accredited School of Nursing

Must be licensed in Alaska as a Registered Nurse

One or more years of experience in community/home health agency or in a hospital settling is preferred

Must have knowledge of Medicare and Medicaid guidelines

Must have a working knowledge of home health care and the principles and techniques of professional nursing and required documentation that pertains to it

Should be skillful in organization and in the principles of time management and have knowledge of management processes

Must be able to contribute to the quality of care being rendered through constructive communication with nursing managers and staff

Must have a fingerprint criminal background check

Speech Therapist

Job Description:

Provides skilled speech pathology service on an intermittent basis to patients
in their homes in accordance with the physician’s orders.

Job Conditions:

The ability to drive and the ability to access patient’s home which may not be routinely wheelchair accessible are required.

Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and to perform patient care.

On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds.

Qualifications:

A Master’s Degree in Speech-Language Pathology with a Bachelor Degree in the Field of Speech and Hearing Therapy preferred.

Preferably a member of the American Speech-Language and Hearing Association.

Must be licensed in the State of Alaska.

Must have at least one year of institutional experience.

Must have a fingerprint criminal background check.

Must have current CPR certification.
